RNFOO Application Submission Guidelines
THE APPLICATION PROCESS IS NOW CLOSED
• Applications must be completed by Monday, January 23, 2012, 4pm EST.
• All required transcripts and letters of reference must be received in the RNFOO office by Monday, February 6, 2012, 4pm EST for your application to be considered
• You can complete your application over multiple online work sessions. Input must be in English
• Please review ALL of the guidelines below before starting the application process
• For your information, here is a description of the application review process that will be followed by the RNFOO Awards & Scholarship Committee judges
Application Checklist
1. Review the Awards and Scholarships brochure and the Eligibility Guide to identify the award(s) and scholarship(s) for which you are eligible. Please be sure to apply only for those award(s) for which you are eligible. All applications for an award for which the applicant is not eligible will not be reviewed.
2. Once you have decided you are going to apply for a 2012 award(s), you must register to apply, and pay a one-time non-refundable application processing fee of $30 via PayPal (or $20 if you are currently an RNAO member). Only one fee is required per applicant, irrespective of the number of awards for which you are applying. (Please note: the application processing fee is not required to apply for an Infection Control Award, such as a SARS Award or a Rolling Stones/CPI Award. 3. Career goals summary – no more than 500 words - refer to guidelines for Career Goals Summary Instructions.
4. If you are applying for a Research Award, refer to Research Proposal Instructions.
5. A resumé in the correct format as per the Resumé Guidelines.
6. If applicable for the award for which you are applying, submit a list of your publications and/or completed research from within the last five years. (This information can therefore be excluded from your resumé).
7. If you are using the RNAO discount rate of $20 for the RNFOO application processing fee, you will need to enter your RNAO membership number. (Not sure if you’re an RNAO member? This is what an RNAO membership card looks like).
8. Please ensure you have contacted your two references. Please review the Reference Letter Guidelines.
9. Brief Biography to be used for RNFOO publications – no more than 100 words – please use the “third person” when writing. (View how winners in previous years have done this at Past Awards & Scholarships Winners).
10. When you have entered all of your information and are ready to submit your application, go to the STATUS tab (visible when you have clicked on Complete Your Application) and set the Application Status flag to Complete. This will ensure your information will be reviewed after the January 23rd submission deadline.
11. You will need to submit proof of education enrollment. Acceptable documentation consists of an official letter from your university/college stating you are currently enrolled in the program, or when you were accepted into the program. Transcripts or proof of payment are not acceptable proof.
12. Contact your university/college to send your official transcript directly to the RNFOO office. Do not fax your transcript. Please allow two weeks for the delivery process. If the transcripts are coming from outside of Ontario, allow 2-4 weeks, and if from outside of Canada, allow 4-6 weeks.
The closing date for the 20012 Awards & Scholarships Applications is Monday, January 23, 2012, 4pm EST, after which the online application process will be unavailable. Please note that all required transcripts and letters of reference must be received in the RNFOO office by Monday, February 6, 2012, 4pm EST for your application to be considered. (You will be contacted by RNFOO if your application is incomplete as of the deadline within 48 hours of the February 6th deadline, such that it will not be considered – there is no need for you to call RNFOO to check).
