Academic Credentials

Despite having a BSc, two Master’s Degrees( Nursing and Medical Education), and a PhD in Nursing ( all verified by World Education Service), I completed the post-RN program at York University to get familiarized with the Canadian Context and get my license as an RN. I took a course at St. Michener Institute to prepare for certification as a diabetes educator. I became a certified diabetes educator in 2022 (Certification Number; 0522013507). I have completed a fellowship program at UHN (a joint program with VHA) and conducted a quality improvement project. Lessons, I learned during this 6 months program helped me to write the application for this innovation program. The results of the indicated quality improvement project were published in a highly credentialed journal of wound care. (Moattari M, King EC, Ruco A. Whole versus hole: enabling community nurses to implement holistic wound care. J Wound Care. 2023 Nov 2;32(11):748-757. doi: 10.12968/jowc.2023.32.11.748. PMID: 37907360.).

Statement Supporting This Application

To Whom It May Concern
I, Marzieh Moattari, am a licensed Registered Nurse and a Certified Diabetes Educator. I would like to apply for Diabetes Nursing Interest Group: Special Projects in nursing award.
My professional career backs up my experiences in the field of diabetes as a nurse/ clinical educator and researcher (with published research) in the field of diabetes. For example, as a research interdisciplinary team member, I facilitated the collaboration in the group to design, implement, and evaluate a virtual diabetic education program for nurses who work with clients with diabetes. Additionally, I have applied adult learning theories/principles in the teaching of nursing students and clients with diabetes in either individual or group sessions. As a former teacher, I had a chance to teach chronic diseases and even conduct research to promote the health of individuals living with chronic diseases such as Cardiovascular and Nephrology (Dialysis). Although I am proficient in the Persian language, my research papers have been published in English.

I have worked as a home visiting nurse and clinic nurse in Canada since 2018. In this position, I have had a chance to provide care to different clients who had already been diagnosed with pre-diabetes and/or diabetes or had been experiencing the metabolic syndrome. The unique nature of this clinic provides me with an excellent opportunity to assess clients comprehensively, identify their learning needs, and motivate/ empower them to apply self-management strategies by incorporating motivational interviews and change theory to improve their lifestyle choices including exercise, diet, stress-management, blood glucose monitoring, adherence to the treatment regimen, travel management, and medical emergencies. I usually tailor the education to everyone’s learning styles, health literacy, cultural values, motivational factors, and social determinants of health.

Many of my clients are newly discharged from the hospital and may be unaware of the need to reconcile their medication regimens. I have supported/ educated these clients on the importance of taking prescribed medicine or consulting with physicians to re-adjust their medication. I usually explain the effect of hospitalization or other situations on Blood Glucose (BG) and the need to monitor and maintain BG in range to prevent or delay diabetes-related complications. I follow up with them about screening tests to address complications such as cardiovascular disease, stroke, retinopathy, nephropathy, and neuropathy.

Many of our clinic clients live with foot ulcers and are at risk for amputation. They need a support network of family or friends to help with the management of the ulcers. As a nurse, I have encouraged and supported family and friends’ engagement in teaching-learning sessions to equip them with wound care management skills

I consider equipping these current and future clients with tools to support the management of diabetes, identification of risk factors, and prevention of complications. I want to build on these core competencies in my new role as a certified diabetes educator. Thank you for your consideration of my application.
Sincerely,
Marzieh Moattari (signed) 6/3/2024
